After Action Reviews: How to, and templates

A few years ago I was working in a small, slightly freeform department within a large media organization. One of the things I ended up doing was setting up a process for running project debriefs - or After Action Reviews.

Debriefs recently came up on a mailing list I read, and I offered to pass on both the process I derived for the department, and the document template for the writeup involved.
